Two different things are shown — please don't conflate them:

• SAUHMA Grade A / B / C is the strength of the society recommendation for the indication — how strongly HBOT is recommended, not the design of any single study. An indication shown without a grade badge is one for which no SAUHMA A/B/C grade is on record — typically a composite or multi-condition indication the society has not graded as a single entity. A grade is a society determination, so none is inferred here; absence of a badge is not a judgement about the strength of the evidence, which is set out on the cards themselves.
• The study-design badge on each source (RCT, systematic review, meta-analysis, cohort, guideline…) reflects the methodological hierarchy of the evidence. It indicates design only — it is not a formal GRADE appraisal.
• The number beside each indication is a count of appraised records, not a measure of evidence strength. A few strong, on-point studies can matter more than many heterogeneous or overlapping ones — read the qualitative appraisal on each card.
